R.E.A.L. Outreach is our frontline clinical outreach program, built to engage youth and families in real time — before, during, or after crisis.
We reach youth through a combination of community-based outreach and referrals from schools, service providers, and families. Whether we meet them in the field or someone sends them our way, our goal is the same: build trust and connect them to real care.
Our outreach team delivers Community-based behavioral health services, including:
Behavioral health screenings and initial clinical engagement
Referrals to therapy and other support services
Case support and crisis navigation
Mentorship, follow-up, and goal-setting

Short-Term Residential Therapeutic Program (2026)

St Claire Legacy Housing
We are developing a state-licensed Short-Term Residential Therapeutic Program (STRTP) to serve youth and young adults ages 5–21 who require intensive mental health services prior to family reunification or transition to independent living.
The program will operate 24/7 and offer a safe, trauma-informed residential setting with consistent care, clinical supervision, and individualized behavioral health support.
Each residence will be age-appropriate, designed to meet the unique developmental and emotional needs of different age groups — from early childhood through adolescence and young adulthood.
Core components will include:
Individual, group, and family therapy
Psychiatric assessment and medication support
Crisis stabilization and behavioral interventions
Life skills training and daily structure
Academic and vocational coordination
Discharge planning and long-term placement support
Our first STRTP site is targeted for 2026, establishing a therapeutic bridge for youth in crisis to heal, stabilize, and thrive.
